Expertise Thesis Abstract;

Mahramli Gizem. Effects of Immediatric Implant Placement and Provisioning on Soft and Hard Tissues: A Systematic Review. Istanbul University Faculty of Dentistry, Department of Periodontology. Speciality Education Research Thesis. Istanbul 2023.

Objective: The aim of this study was to evaluate the effects of emergency implant placement and temporary implant placement on soft and hard tissues in a systematic review of randomised controlled clinical trials with a follow-up period of at least 12 months and to create a reliable source of information that can guide clinicians.

Materials and Methods: This systematic review was written by applying the items in the PRISMA 2020 ‘Preferred Reporting Items for Systematic Reviews and Meta-Analyses’ checklist. With the selected keywords, a detailed literature search was performed in original research articles published in English in MEDLINE (PubMed), Wiley, Turkish Citation Index databases from 2015 to May 2023. In this systematic review, changes in bone and mucosa were considered the primary outcome variable. Aesthetic evaluation was considered as a secondary outcome variable. We ensured that the studies selected for inclusion in the systematic review included data on these variables.

Results: A total of 2250 studies were accessed by performing a detailed search with the selected keywords. After reviewing the title, abstract and full text, it was decided to include 7 randomised controlled clinical trials that met the inclusion criteria for systematic review. In 3 of the 7 included studies, each patient was immediately implanted and a provisional restoration was applied in the same session in the test group, while no provisional restoration was applied in the same session in the control group. In the other 3 studies, implants and provisional restorations were applied immediately to each patient in the same session and subepithelial connective tissue graft was used in the test group, but not in the control group. In one study, each patient received immediate implants and provisional restorations in the same session, surgery was performed without flap removal in the test group, and surgery was performed with flap removal in the control group. In immediate implant placement, soft tissue loss was observed independently of provisionalisation. In studies with a stable volume and connective tissue graft, the mucosa level in the soft tissue was observed for a longer period of time.

Results: Immediate implant placement and provisionalisation can be considered a treatment method with reliable and predictable results, regardless of whether an additional connective tissue graft is applied. Randomised controlled clinical trials with longer follow-up periods and standardised measurement methods and measurement timings are needed for proper systematic reviews and meta-analyses.
